New to District Students

Welcome to Dunlap CUSD #323

Below is a checklist of items, as well as a list of forms, that must be

completed and provided when you register your student(s) in Dunlap CUSD.

You will need to bring with you the following items when registering:

• Copy of the certified birth certificate (or passport)
• Proof of Residency.
           ○ One item from category A and two from category B
           ○ Examples of residency documentation can be found using this link.

• Copy of legal guardianship (if applicable)
• Copy of custody papers (if applicable)
• Copy of health records – immunizations and most recent physical

The following forms are available on the grade specific buttons below and must be filled
out and returned at the time of registration.

• Registration forms
• Home Language Survey
• Release of Records

Once this has been turned into the home school, you will receive an email with a
student specific Snapcode to complete the final step of registration.

New Student Registration Requirements

• For either Kindergarten or New Student Registration, you will need the following information:
• Original, certified copy of your child's birth certificate (no photocopies will be accepted)
• Three Proofs of Residency (More information here)
• A current Illinois physical, dental, and vision exam (by the first day of school)
• Students entering from out of state need a current Illinois physical and dental and vision exam

Access to the online registration portal cannot be provided until three proofs of residency and their child's certified birth certificate are on file.

Pre-registration forms are only needed for NEW students. Returning students do not need to fill out Pre-registration forms.
